Establishment clean...met by Silvia the concierge when I arrived. She went out of her way to get to make me comfortable and set up tasks i needed fulfilled. Doctors, Casey manager Angel and p/t staff were very helpful...something can be said for the food...aides could use a lesson in personality and courtesy.

Should be 0 stars. My husband was transfered on Wednesday 7/22/26 around 6:30pm. The room was filthy, had not been cleaned after previous resident left, there were still personal items in the room. A staff member did a very quick wipedown of a fe things and left the purple wipes telling me I could use them to clean whatever else I felt was necessary. We were told the room would be cleaned first thing in the morning. It was still not done by 2:30pm. We were told by admissions there would be a pneumatic cardiac walker in the room for my husband (he had been using it in the hospital for the…

Should be 0 stars. My husband was transfered on Wednesday 7/22/26 around 6:30pm. The room was filthy, had not been cleaned after previous resident left, there were still personal items in the room. A staff member did a very quick wipedown of a fe things and left the purple wipes telling me I could use them to clean whatever else I felt was necessary. We were told the room would be cleaned first thing in the morning. It was still not done by 2:30pm. We were told by admissions there would be a pneumatic cardiac walker in the room for my husband (he had been using it in the hospital for the previous 22 days) and were then told they didn't have one. They said they would have all my husband's medications available. They did not have the dilaudid he was switched to on the med list but there was percocet on the list, along with his clonazepam. They told him that the meds weren't available though I confirmed with the director of nursing on 7/23 that they were on the list. They did not give my husband his blood pressure medication (it was not on the list) but they did not tell him that or contact the doctor's office. He had his call light on for almost an hour and someone kept coming in and turning it off. I went on Thursday 7/23/26 to remove him and take him back to the hospital. When I took him to the ER his blood pressure was 155/100 and his pain was severe. The director of nursing apologized and agreed the room was not okay and that he would look into the other issues. The pictures do not show how bad the room was. The "clean" bedding he was given was stained and looked like it hadn't been washed. Like I said, the Director of Nursing apologized and offered to "make it right". However, I absolutely could not trust the care as what I saw was awful, what do the areas I couldn't see look like? Also, I wouldn't trust the staff to care for my husband after my complaint - I wasn't rude or nasty, nor did I yell but I was 100% upset and disgusted. There are 2 other facilites under the parent company, Capri Health & Rehab in Venice & North Port Rehab & Nursing Center The caseworker at Sarasota Memorial hospital also tried calling the VP and never received a call back. I wouldn't put a family member in any of these facilities. EDIT: If you read the review, we did speak with the director of nursing at the facility. The caseworker at Sarasota Memorial Hospital tried contact your VP and didn't receive a response. You can't make it right. How can I trust you with my husband's care at this point? The room was filthy, medication on the list provided by the hospital wasn't provided to my husband, his blood pressure was high and either no one took his vitals or didn't care. I know that things happen and if it had simply been one medication that I or my husband needed to call the hospital to send over orders for, that would have been fine. If this happened to us, where my husband is in his early 40s and has no cognitive issues, how bad is it for those that can't advocate for themselves or don't have someone to do it for them?

Absolutely find another place for your loved ones.understaffed..not clean..patients not fed.or showered regularly. Not able to meet with Director To discuss any of our concerns..always Not available. We chose to discharge our Mom and remarkably in a new facility with care..medication review and therapy.is now living her best quality of life.

If your loved one has no other option and must go to this facility for care be prepared to visit daily and advocate for basic needs. I fear for those that do not have someone that is able to check in on them to be sure they are being properly taken care of.

I toured lots of facilities before deciding on crescent and I can say both my parents were providing with good care and the facility took any concerns and quickly rectified them. The nurses and aids care about the residents and many go above and beyond in caring for them . I want to say Vanessa in charge of nursing services was amazing she goes above and beyond in answering questions and working with me to provide the best care for my parents . Physical therapy was also excellent they got my mother back on her feet and mobile again. If I had to use a rehab or nursing home facility again I would return to crescent .
