St. Augustine Health and Rehabilitation Center

My father, Frank, was here for a couple weeks for PT. The staff is 98%super and does a great job with no issues. Kudos to Mia, Glenda, Melissa,Noah, Simone, Sara, Tarnisha, Kim, and many more. The food is acceptable, most meats are ground up and so not too great, the rest is good. Alternatives are available on request and include grilled cheese, hamburgers, chef salad, and are really well done. Thumbs up on the grilled cheese!! It’s an older building but adequate. Therapy is very well done. 24/7 controlled entrances and exits provide security. My father cut his leg severely climbing into my truck, and staff was on it in seconds stopping the bleeding and bandaging. It was not at all the facility responsibility for the injury, not at all. Went to ER and 19 stitches later he is on the mend. The staff has done the wound care follow up cheerfully and professionally. Many thanks for all the great treatment and attitude in every situation!!

I had to place a star in order to make a review - otherwise it would have a no star review. I had my mother there THINKING it was a good place since it is near my home. It actually was the worse place and I would never recommend. My mother had a single room and had an emergency while there that she had to be seen at Flagler Hospital. She was gone for a couple of hours and when we got back they placed another person in her room - my mother’s belongings were packed at the door and several of her items were stolen. I had to go to laundry room to where none was found. When I went the next day to make aware to the manager she wasn’t listening and just kept eating her lunch with her husband as if she could care less. Also the nurse that past meds didn’t wear gloves and was handling medication in an unsanitary way. Not once did the manager help me find or approach the missing (expensive) clothing my mother lost

The whole leadership team is super helpful and has the patients best interest at heart!

My dad stayed here for 5 days while I had respite through hospice. He has alzheimers and was taken care of fairly well while here. I visited him everyday but one and would send him here again if I was able. I have been taking care of him for 5 years at his home and this was a good break for both of us. This place is clean, staff are friendly and approachable.

We love the expert care and compassion we receive from the staff here at St. Augustine Health and Rehab. We highly recommend this facility to any family in need of long-term care.
