About this community

Sandalwood Rehabilitation and Nursing Center is in Daytona Beach, Florida, on South Beach Street near the Halifax River and downtown. Its listed services focus on skilled nursing and rehabilitation, including short-term post-acute rehabilitation and long-term nursing care for people with complex medical needs. The short-term option is described as post-acute care, while long-term nursing is listed for ongoing needs. One listing places downtown about a mile away; neighborhood descriptions also note waterfront dining and restaurants in the downtown area. The available descriptions do not specify typical stay lengths or admission criteria.

Skilled nursing care is described as available 24 hours a day, with licensed nursing and therapy staff. Physical and occupational therapy are specifically named as rehabilitation services. Individualized care plans are reported to be developed with residents, their families, and referring physicians. The descriptions identify who participates in planning but do not detail treatment schedules or promise particular clinical outcomes. They also do not specify how therapy services are coordinated with individual stays.

Nursa also lists memory care and respite care among the center’s services, though its description does not explain how those services are arranged. Published listings give separate room and capacity figures: one reports a 99-bed capacity, while another describes 67 private rooms. They do not explain room configurations or how the figures relate. A facility listing mentions resident activity programs and outdoor spaces, as well as renovations, but gives no details about the activities, outdoor features, or renovation work.

The center’s South Beach Street location is near the river and downtown Daytona Beach. The Halifax River Esplanade is identified as a nearby waterfront park along Beach Street, with walking paths. This public park is distinct from the outdoor spaces mentioned in the facility listing; the sources do not say that they connect or share facilities. The available information also does not indicate whether residents have organized outings or transportation to nearby places.

Listings identify Halifax Health Medical Center and Florida Hospital Memorial Medical Center among the hospitals in the area. They do not establish which is closer or describe hospital services as part of the center’s care. Rather, the hospitals provide context about nearby medical facilities; the sources do not explain referral or coordination arrangements.

SeniorAdvice reports that the facility is certified for both Medicare and Medicaid and has participated in Medicare since 1967. This provides background on its reported certification alongside the listed nursing and rehabilitation services, but does not establish what services are covered for any individual resident. The available descriptions do not provide unambiguous information about dining or in-room amenities, so those details are not included here.

Mary has been so incredible! Helped me with a few things to ensure my mom was discharged appropriately and has followed up a few times a week to check to see how she is doing. She was well taken care of at Beach street. Thank you so much in assisting in getting her back up on her feet after a major heart attack.

My husband Alex was in this facility and they treated him like a king. The staff was wonderful Rose was one of his CNA'S, Sarah was another one of the nurses but everyone was wonderful. I just spoke to Holly and she's going to help me get someone else in there that is being abused at another facility on 3rd Street which should have a negative star rating. If you need to put your family member in a facility for rehab this is the place. I give them a five-star rating !!!

The staff was friendly and seemed like they genuinely cared. My dad said the food was pretty good. The only downside was that there was a courtyard area however there was only certain times he was allowed to go outside.

I don’t leave reviews, but I had to after what I experienced with my friend. I am power of attorney for one of my Friends and the communication I requested versus what I received was terrible. My friend was wanting to sign himself out AMA, I discussed with the nurse desk what would be needed in order to allow him to do that, and was given completely inaccurate information. He ended up having to stay three additional days because he did not have somebody that could take him home. (I was not told and had left town). I understand this is protocol, and misinformation to someone’s power of attorney and loved one is unacceptable. My friend asked for his beard to be shaved, and instead they shaved his entire face, head, AND his eyebrows? When he confronted them about it, they laughed at him. Who does this to an elderly person? The food looks like it could’ve came from a prison. Granted,I can’t knock them completely for that. It’s not the staff’s choice of what food gets served. I wouldn’t touch that food with a 10 foot pole. There were some good people in there, but majority of the staff I feel is ill equipped or will give you attitude with requests made. I would NOT recommend this facility to anyone or their loved one. I hate to think this is what our medical model has come to.

Our assigned health coordinator was/is outstanding. She has proven to be a great help and a much needed resource.

The staff here were excellent and took great care of my father. We are forever grateful for their services.

Friendly staff, Great food, and communications! My brother was there for a short time and wanted to live full time. They didn’t have any long term rooms available but I’m glad he had a good experience!

Very concerned staff.
