If you are looking for edible food forget this place. Rooms are disgusting once you get in and look around. I mean dirty. Furniture is all beat up and old. Rooms haven't been painted in 30 years at least. My mom saw roaches in her room as well. I couldn't wait to get her out of there. Admin really needs to address the housekeeping issue it's very bad. And the food is pathetic. My mom's bathroom was cleaned once in 5 weeks. And they…

If you are looking for decent physical therapy this place is okay. If you are looking regularly cleaned rooms and bathrooms or garbage removal forget this place. If you are looking for edible food forget this place. Rooms are disgusting once you get in and look around. I mean dirty. Furniture is all beat up and old. All the employees I've met are so polite and courteous, they care alot about their patients. They listen to any concerns that you may have, if I had to recommend this place I would hands down. Thank you for all you do.

My wife, who has dementia, has lived in an ALF/Memory Unit for the last 2 years. On June 6, she was admitted to Mease Countryside Hospital for treatment of acute UTI. She was sent to the Oak Park Rehabilitation Facility on June 14 for rehab after a week’s stay at the hospital. She has been given excellent care and has participated in many rehab therapy sessions. The nursing staff watch over her meds and have suggested and implemented changes (after consulting with me) in her meds because they thought she was being over medicated. The cleaning and maintenance staff do an excellent job of keeping the facility clean.
my grandpa needed rehab and this facility made sure he was taken care of . Karina the social worker gave us all the info needed and gave us resources for home and my family really appreciated. she made my family feel so welcomed. would recommend OAKPARk!!
My companion was brought to Oakpark Health & Rehab May 19, 2026, and not to happy about it. When I got there today I was so relieved. Carlogio was happy. Everyone was so helpful, a refreshing scent upon entering. Exceptionally clean. I will definitely sleep tonight. Thank You! Everyone. Dawn Lewis
My husband was admitted to Oakpark for rehab after open heart triple bypass surgery. He received Physical, Occupational and Speech Therapy. He was severely weakened after the surgery. Physical Therapy worked diligently with him to safely reach independence with a walker. Occupational Therapy greatly contributed by emphasizing both upper and lower body strengthening which especially protected his internal surgical site promoting healing. Speech Therapy quickly detected the need for work on his recall and memory related to Parkinson’s Disease affected by post operative side effects. The nursing staff especially Adam, Ben and wound care nurse, LuLu, were professional, attentive, and proactive employing critical thinking in many aspects. Myla, the supervisor, was an invaluable resource as she was empathetic, attentive and always available addressing and ensuring all my questions and concerns were answered in a timely manner. Administration from Su Soriano to Social Services instrumented a seamless transition from admission to discharge.
Tha staff was great and the nurses taken care of my father are very passionate and genuine about thier work.
