About this community

Isle Health and Rehab Center in Orange Park, Florida, provides post-acute and long-term care in a skilled nursing setting. Its listed services include skilled nursing and rehabilitation, and the center’s website describes specialized cardiac and pulmonary care. A third-party facility profile also lists memory care, though the center’s own pages do not detail that service. The available information describes both care for people recovering after a hospital stay and ongoing support for people who need long-term care.

Rehabilitation is described as individualized, with treatment plans tailored to each person and therapy offered seven days a week. A third-party profile names physical, occupational and speech therapy among the services. The center presents rehabilitation as part of a transition from hospital to home, while its descriptions focus on supporting recovery and independence rather than promising a particular outcome. Cardiac and pulmonary care are also identified as areas of specialized care.

For long-term care, the center describes around-the-clock support and an emphasis on personal choice and independence. The information does not specify particular room types beyond private accommodations. Its website describes rooms as furnished with amenities and intended to provide a comfortable, home-like setting. These details may be useful when comparing a nursing setting with a person’s preferences for privacy and daily support.

The center’s guest-services information includes dining as part of daily life. A third-party listing describes meals prepared by a chef and served in a dining area, with attention to dietary preferences and restrictions. That profile also lists air conditioning and television in rooms, along with laundry and housekeeping services. The available descriptions do not specify individual meal schedules or how particular dietary needs are handled.

Activities listed by a third-party profile include art classes and educational, social, recreational and entertainment programs. It also names a library or reading room and a fitness center among the amenities. The descriptions indicate options for spending time in shared activities or using these spaces, but do not provide a schedule or explain how programs are adapted to different care needs.

Planning for a move out of the center is another part of the rehabilitation information. The center describes personalized transition planning, home-safety evaluations and continued support after discharge. An admissions conversation is described as the first step, with the team listening to a person’s concerns and goals before creating a personalized plan. The center also describes time to settle in before working toward a transition home. Guest-services representatives are described as available seven days a week for questions and visit arrangements. A third-party listing also says transportation is provided for medical appointments, social outings and community events. Together, these practical details cover support during rehabilitation as well as services and activities associated with residents’ everyday routines.

My father was a 100% combat-disabled veteran who survived two wars, yet he was treated with a complete lack of dignity and respect during his two months at this facility. More alarmingly, caregivers were verbally and physically aggressive toward my father, who explicitly told us he was afraid of the staff mistreating and mishandling him. The situation became so hostile that I had to call 911, resulting in the Clay County Sheriff's Office intervening at the facility. When I asked staff members to treat my father with basic respect, two caregivers verbally attacked me and called me a "troublemaker." Furthermore, despite our providing full legal documentation, our repeated requests for his medical records were ignored for over 60 days. My father passed away suddenly, and three months later, this center has never reached out or offered a single word of condolence. I strongly urge any family looking for long-term care to do thorough research and look elsewhere to protect their loved ones.

Having worked in the home health care industry for the last 17 years I have been in and out of several rehabs/post acute facilities and nursing homes meeting families and working alongside staff to ensure a smooth transition from hospital to rehab to back a home. Its always been an honor to work with the Isle and their professional team. They make the transition easy, knowledgeable and there is a dedication to excellent service for both for patients and their families. I work the most often with the Social services Director Sumaira McCloud and can attest that the Isle really has a treasure in their Social services department. Sumaira is a true pt advocate: focusing on safe discharges and transitions and educating pts/families on the best services and resources available in the community. She is thorough and detailed in her expertise and this goes along way for the families/pts trying to navigate the ever changing health care industry. Highly recommend this building for families looking to receive additional nursing and therapy services after a hospital stay or for respite care for a loved one.

I arrived at 11:45 am on a Friday after a full hip replacement. I was told there was no walker or port-a-potty available for the entire weekend. They were locked up in a room for the weekend !!! I asked to go to the toIlet. They brought in a wheelchair that was broken. I had to hold onto the back, but when I was trying to walk back to my bed I fell 😩 because the wheelchair would not lock!!! The attendants went to get a lift machine. They lifted me up and the battery on the machine died. So I'm hanging in this lifter between the floor & my bed. They found a new battery and finally got me back in bed. I had to have my husband bring a walker and a port-a-potty from our home since that's what my doctor told me to get before my hip replacement. I was sharing a room with a curtain between me & the other person. She had the TV on loud 24 hours a day & night. I asked if she could lower the volume so she put it even louder!!! I asked the night nurse how many patients leave before their week of rehab is complete. She said about 50% of patients leave after 1 or 2 days. This place is dangerous!!! I left the next day in a wheelchair ♿️ as fast as my husband could get me out of there !!!!!!!

Wonderful place to send your loved ones. Samaira from social services is so helpful. Great five star facility!

I truly do not see how this place has so many 5 stars. Just from the smell alone, it should get NO STARS. This place is HORRIBLE. My mom legitimately came out worse than she went in. She fell during therapy which the worst part is they never even did x-rays even after i asked TWICE because her foot was horribly bruised and she was unable to put any weight on it afterwards. There was bugs....mainly ants and roaches which is not a surprise considering how nasty it was. They would let her sit for hours in her own urine and feces. She went from the hospital being able to use a bedside potty to basically an infant 100% dependent on diapers because that was their solution so they did not have to clean up her soiled sheets when it would take them a hour after she called for help to use the restroom. She has had a few episodes since her stay here which resulted in going to a few other short term rehabs that were amazing but anytime they asked where she wants to go, I start off with Isle is absolutely NOT an option! She will NEVER GO BACK TO THIS HORRIBLE HORRIBLE PLACE....... EVER! this place seriously needs to be SHUTDOWN and how they are still open is honestly beyond me!

We were very lucky to find Isle for a long term care facility for my 73 year old sister. Kathy is well taken care of. All the staff working at Isle take pride in there jobs and treat each resident like family. They were an answer to our prayers.

“OUTPATIENT PT IS GREAT AS WELL” My father has been a long-term patient at Isle Health and Rehab Center for over a year and everything has been great. It feels like a big family. The in-house Physical and Occupational Therapy at Isle is the best I’ve been around and Emily Starks has put together a team of all-stars. Just recently my mother was in need of PT and the process to receive Outpatient PT was so smooth and easy. I am surprised that there are not more people taking advantage of the Outpatient PT that is offered at Isle HRC.

Great staff will work with you, help family and patients best, and will go beyond to help.
