2.9 · 29 reviewsPlease don't put your loved one here. If they can't fight for themselves and you can't be by their side 24 x 7 then it's a huge problem. Communication between the staff and the patient's advocate is horrible. You'll not get information you need when you ask and when you do you're gonna get the run around if you question them. They're right and you're wrong. So sorry we put my family member in there and it's now a battle to get them out. Food is always cold when it gets there and feeding is an issue.
The hospital sent my friend to Boulevard Rehabilitation Center. It was terrible. He had a feeding tube in him, and I saw nurses dropped the feeding tube on the floor, picked it up, and stuck it back into his stomach. I had also seen the oxygen that went into his nose fall on the floor. They picked it up, and put it back in him. They also lied to me. I live in Florida. This was in December, so it was a very cold day. I went into the room, and the room was freezing cold. My friend was shivering and shaking. I said to the nurse, 'Something's wrong because it's freezing cold in this room.' They…

The hospital sent my friend to Boulevard Rehabilitation Center. It was terrible. He had a feeding tube in him, and I saw nurses dropped the feeding tube on the floor, picked it up, and stuck it back into his stomach. I had also seen the oxygen that went into his nose fall on the floor. They picked it up, and put it back in him. They also lied to me. I live in Florida. This was in December, so it was a very cold day. I went into the room, and the room was freezing cold. My friend was shivering and shaking. I said to the nurse, 'Something's wrong because it's freezing cold in this room.' They moved him to the other side of the room, where there was an air conditioner like something you would see in a motel. Then another time I went in there, and the room was stifling hot. It was because they pushed the bed up against the air conditioner because he was a fall risk. They didn't want him to get out of the bed. My friend was there for 15 days, and he was on pureed food. The facility was clean though.

There is a full time infection control staff and daily dialysis offered for patients who need it in house.

I visited my uncle here many times. Very clean, staff is nice and attentive. There is no smell. My Uncle even loved the food. The staff and I got along very well, my Aunt was very comfortable leaving. My Uncle enjoyed talking to the staff about his army stories, even thought it was repetitive no one seemed to mind. It seems to me everyone was easily approachable if we needed anything. The rooms are kept clean and bright. The fact that we could walk in any time and find no issues was such a relief to me and my family. Thank you all for caring so much and loving what you do. I would recommend this facility to anyone, as a matter of fact I have.
The WORST facility! Don’t send your loved ones here. There is no patient care no care for the well being of their patients. Dirty, poor service and neglect.
Very clean facility. Staff is extremely polite
These people are horrible ,they won’t feed my grandfather they forget his trays.They keep him on the same clothes.He is bedridden now due to the lack of care .He got bed sores and caught a infection due to the lack of care.The staff don’t care about the patients .They hang up when you call.The worst place ever,I would never recommend this place to a ant .It’s just the worst I ever seen .
STAY AWAY from this place. It is the worst I have ever seen. I was in there in person every day and the lack of care is total incompetence at best. I would not put my worst enemy in this location. RUN! I understand being short staffed but then stop admitting more people than you can handle. BAD care is worse then no care since you are doing more harm. CLOSE the place if you can't handle the work.
